P17, 1-16: The initial incident report was broadcast at approximately 6:12 and the agent reports to the scene in Butler.
P22, 16: “I never went inside the building. I never went on top of the building.”
P23, 13 to 18: Trying to trace the firearm and the shooter.
P-24, 22 to 24: Traced back to Thomas Crooks.
P25, 23 to24: Decides to travel to the other scene in Bethel Park.
P26, 10 to 12: By 9:20 pm agents were dispatched to Bethel Park to set up covert surveillance.
P28, 5 to15: Received email at 10:28 “am” (corrected later) containing Crooks’ drivers license, and employment report.

P28, 24to 25: Compared Drivers License picture to profile of Crooks taken at Butler.
P-40, 9 to 10: Crooks’ father, Matthew, identified as original purchaser.

P41, 24 to P42, 9: Comparison of Matthew’s license, Thomas’s license, and the ID picture of Crooks taken on the roof by an unnamed law enforcement officer. Shooter determined to be Thomas to “a high degree of certainty”.
P43, 8 to10: They learn Matthew has called 911 about 11pm.
P44, 3 to14: Report that the father came out of the house saying the news had contacted him. Agrees to let agents search the house. Mother is escorted out since she is visually impaired. Suspected device found in Thomas’s bedroom. Evacuated houses on either side.
P45, 17 to 21: Crooks’ Hyundai Elantra has not yet been discovered.
P46, 9 to 11: On the way to Bethel Park, agent finds out Crook’s car has been found.
P46, 15 to 25: Arrives in Bethel Park between 2am – 2:30 am. House has already been cleared.
P47, 9 to 16: Sees the parents, gives card if they need anything. They have questions. Agent says he’s not there to interview them.
P47, 17to 19: Father doesn’t want the gun back, he just wants to know where his son’s body is.
P48, 4 to7. Recalled to Butler by 10 am. Day is very hot and humid.
P49, 12 to 21 Who knew about the linkage between the Crooks family and the shooter before 11 pm on the 13th? The IRS agent, the special agents in charge, the people on the email forward from the agent, FBI and ATF. No Secret Service.
P49, 24 to P50, 6: How did the 911 call get to the agent? Agency was already in touch with Bethel Park PD.
P51, 3 to P53, 25: How they found the Hyundai Elantra, discovering and detailing the contents of the IEDs inside, and if they might have worked.
P54, 16 to P-59, 22: More detailed recap of surveillance of the house, exhausted condition of the parents during the interview, and Matthew detailing the education and engineering programs his son was involved in. No behavioral problems. Worked at a retirement community in the kitchen for a couple of years.
P59, 23 to 25: Father had concerns that the son was at the gun club and said, I should have known better. He was trying out for the 150 yard club, which at a rifle club is not out of the ordinary.
P60, 1 to 4. Crooks had been going to the gun range for some years, previously with his dad. Had also taken courses at Keystone range.
P60, 7 to 16: Did Crooks have any political leanings? He never made any statements against anyone. No major political motive.
P60, 20 to P61, 10: Discussion of the device found at the house.
P66, 19 to P67, 2: Negating conspiracy theories.
P69, 17 to P70, 4: The Bethel Park residence needed to be secured because surveillance had noticed vehicles driving slowly by. When this information was relayed to the agent in charge, it was noted the address had been leaked to the press.
P70, 6 to 18: Matthew Crooks called 911 at 22:57 to report his son missing.
P70, 23 to 25: Five minutes after the 911 call, Matthew was called by CNN, then another call from CNN, then a call from NBC Universal.
P71, 4 to 12: Physical contact was made with the people in the house approximately 11:40 pm. They receive consent to enter for a protective sweep from the father, and they find the device in the bedroom.
P72, 9 to 11: Crooks mother asks where her son’s body is. Agent said eventually it will be at the coroner’s office.
P-72, 12 to 22: Mentions a recorded interview with Matthew that ended at 1:13 am and turned over to ATF and the FBI.
P73, 6-10: Brief description of a coworker giving Thomas a ride home in 2022.
P73, 14 to P74,1: Condition of the house. Clean, no soapy rags or evidence of cleanup. One bathroom under renovation but the house itself was in neat order.
P74, 3 to 11: General discussion of photos and recordings taken into evidence.
P76, 8 to 14: Discussion that the IED was found at the foot of Thomas’s bed, and that the houses to either side were evacuated.

P95, 13 to 19: Suspicious person outside the perimeter is an issue for local law enforcement. Inside the perimeter is Secret Service responsibility.
P95, 21 to 23: Post standers can report a suspicious person but can’t leave their post.
P99, 6 to 8: “Were you made aware of a suspicious person with a rangefinder?” “No.”
P101, 12 to 23: Notices the local TAC team leave the barn and line up along the AGR fence. First indication something is going on.
P103, 10 to 20: Conversation with another agent . Other agent had said nothing about her pursuit of Crooks. Interviewee agent was on the roof after the shooting, came down and spoke to her again. Other agent asked if the guy on the roof was the same guy as the suspicious individual, but interviewee agent did not know. Other agent sent text message with picture of Crooks. The interviewee agent confirmed, that’s him.
P120, 14 to P121, 13: Discussion of how Crooks got on the roof. Officer assumed it was the ladder, Agent said no, the police just brought that over.
P121, 14 to P122, 8: Post-shooting discussion with Butler ESU officer about how he saw Crooks through a window of the AGR building. He saw Crooks with a rangefinder, but then he moved closer to the building where they couldn’t see him. “He was backed up against the window.” Then he moved to the wall where the agent took the pictures of him. Crooks moved to the front of the AGR building. The officer got the attention of another uniformed officer and told him to find the guy because he was acting suspicious. And that was the last they saw of him.

P11, 8 to 21: Describes morning briefing on July 13. Secret Service was not present. Witness does not know if Secret Service attended the sniper meeting.
P13, 1 to 3: The AGR Building was not discussed in the morning briefing.
P14, 8 to 18, discusses map of sectors at Butler Farm Show. He is stationed in sector 5, at the vehicle gate, between the two barns. They are the first to see the people as they enter.

P45, 10: Shots fired. Patrol channel 3 comes alive.
P45, 12 to16, They hit the lights, make the right, agent opened gate and went to command post.
P45, 17 to P46, 10: Discussion of getting into protective position between the flag cranes and attending to the injured.
P46, 13 to P47, 25: Shooter down. Discussion of images sent from the AGR roof including remote trigger and bag.
P-48, 6: “DOD asked if they could see the bag, agent replied they can go on the roof if they want.
P48, 10 to 11: Took dogs inside the building to look for individuals and explosives.
P48, 19 to 25: detained six witnesses an a possible accomplice under a tree in the southeast corner of the parking tot but had no one to give those individuals to at the moment.
P49, 1 to 7: Discusses bicycle, objective to clear the water tower of any possible accomplices, and turn the gas off in the building. Building is being cleared of people.
P51, 1 to 18: Discussion of radios picked up from Butler Command that morning, and who had what device on them.
P52, 8 to 12: Once you get everybody on one frequency it’s impossible to pass emergency traffic, so they split frequencies.
P52 21 to 25: Pennsylvania State Police do not have access to Butler County radio channels, and no one has access to State Police frequencies. State police might have access to Butler County frequencies.

P71, 1 to 18: Reiteration of the position of the witnesses under the tree after the shooting, turning off the gas, and covering the water tower. First ladder rung to water tower was 15 feet off the ground so they waited for a ladder or drone to check it.
P71, 24 to P72, 2: Witnesses removed by vehicles to Command Post.
P72, 3 to 15: Bomb squad clears the perimeter for possible controlled detonation until the AGR owner shows up with keys to open anything they wanted and they were satisfied. No detonation.
P72, 21 to 24: In lieu of taking notes, a trooper with a body cam was told to follow this person (speaker) around.
P74, 1 to 4: Body Cam trooper was out of Indiana Barracks.
P82, 8 to 11: Officer thought the buildings of AGR were separate and the suspect was running through the buildings to get back to the parking lot, but that’s where he accessed the roof.
P88, 8 to 14: SWAT had helmet cams but they can only get an hour and a half of film off that, so they would not have been turned on all day.

Pg 18, 14 – to Pg 19, 7: Didn’t notice what they were getting into until they arrived at their posts. Expected patrol presence was not there, causing MEUs to participate in incidents not their job, such as a disagreement between vendors.
Pg 22, 20 to Pg 23, 10: Secret Service was supposed to hold a face-to-face briefing with Beaver County ESU snipers, but never did.
Pg. 27, 8 to Pg 28, 22: Describes a Homeland Security Office sitting in his car and running a gate near the ESU team. He was opening the gate to let Secret Service or Homeland Security in and out from behind the stage area.
Pg. 28, 23 to Pg 31, 18: ESU used radios to communicate with each other (six members on the ESU team) and text to communicate with a sniper on the other side of the fairground, but their radios were not tied to the other teams, and this was not unusual. Only the Sargent and Commander had access to the patrol radios that tied into the Command Post.
Pg 33, 2 – 34, 13: Commander received picture of Crooks about a half hour before the shooting. Commander showed picture to the ESU team. Commander mentioned to team that (redacted) had spotted someone and relayed that to command post, and they were looking for him. No communications or updates of any kind came from the secret service that day.
Pg 43, 23 to Pg 44, 5: Received information around 4:45 or 5 pm of a suspicious person. Saw pictures of Crooks from the back and profile.
Pg 46, 16 to Pg 48, 6: description of the moments before and during the shooting
Pg 48 17-22: HSU agent had already gotten out of his vehicle and was opening the gate for them to drive through.
Pg 49, 11 to Pg 53,11: description of their actions at the stands directly after the shooting, attending to one person with a chest wound, then making sure the red hydraulic fluid sprayed was not blood on other attendees. Approached by Washington County ESU and told to go to the AGR building immediately because there may have been a mass shooting there, and also the shooter was down. Medic let commander know they were requesting them to go to AGR, Commander asked them to take operators with them because medics don’t carry guns. Saw officers already on the lower roof of the AGR building. Climbed onto the roof via ladder. Request to evaluate Mr. Crooks. Arrived about 6pm, pronounced him at 18:15.
Pg 53, 12-24: Got on the roof, put on gloves and observed Mr. Crooks. Felt for pulse in carotid artery and there was absolutely no pulse. Stepped off and remained in place, waiting for the person who was going to be in charge of the investigation.
Pg 54, 2 to Pg 55, 20: Description of process for declaring someone deceased, and of body condition, flex-cuffs, and observable exit wound. There was nothing unusual about Mr. Crooks body compared to other individuals medic has seen with gunshot wounds.
Pg 55, 25 to Pg 56, 7: Medic does not know why body was sent to Allegheny County rather than Butler County.
Pg 56, 9 to Pg 57, 7: Had to leave building briefly due to bomb search with dog. Description of checking Crooks’s pockets and finding a detonation device. The dog had also “hit” inside the building so the roof was cleared. Medic never went onto the roof again.”
